As the Medicaid Redesign Team’s Brooklyn Working Group met privately to discuss their plan to redesign the health care system in Brooklyn, Brooklyn State Senator John Sampson convened a group of community stakeholders to envision a health care system from a community, advocacy and provider perspective.
Representatives from SOSC served on Senator Sampson’s Working Group and helped shape the report that provides three central recommendations:
(1) Bolster primary care infrastructure as a first and top priority.
(2) Formulate an active, coordinated care model to streamline collaboration and partnerships between providers currently existing in Brooklyn
(3) Ensure that community-based organizations, and the community in general, are integrated into the health care delivery system in Brooklyn, to promote patient wellness and link patients to the appropriate services at the appropriate time.
We applaud all involved in formulating this report and hope to see it’s recommendations taken into action.
